I haven't seen one like that, but I did have a game a week or so ago where there was a shot, a rebound, and three offensive fouls (one on my opponent, one on me, then one on my opponent) at the same time on the game clock. Seems a little weird.

I'm thinking it may have been because we were both playing fairly fast tactics (motion/full court press vs push the ball/full court press) but still, not quite right.
